FRESH PASSPORT Top
  1. Eleven passport size coloured photographs (front view) 3.5cm x 3.5cm.
  2. Age proof original and photocopy (school certificate or certificate from municipal corporation).
  3. Address proof original and photocopy (telephone bill or electricity bill or bank pass book of present address in applicants name).
  4. No objection certificate from the department in original if applicant is working for any government department.
EMIGRATION CLEARANCE Top
  1. Two coloured photographs (front view) (coloured, 3.5cm x 3.5cm).
  2. Degree certificate in original and photocopy or
  3. Income tax assessment order in original of the last three years or
  4. Provisional degree certificate in original along with the marksheet or
  5. 12th class marksheet in original and photocopy.
CHILD NAME INCLUSION Top
  1. Note: the passport office has stopped child name inclusion service. Child can apply for fresh passport.
CHILD NAME DELETION ( P.c.c. clearance) Top
  1. Two passport size photographs of passport holder.
  2. Original passport and one set of photocopy.
CHANGE OF NAME Top

  2. Declaration in two news papers regarding change of name.

FRESH PASSPORT FOR MARRIED WOMAN Top

  3. Address proof in original and photocopy.
  4. Deed poll affidavit from the court attested by magistrate.
  5. One combined photograph of husband and wife.
